Application Process:
- Requests for application forms will be made to the
sponsorship committee of the foundation
- The application form will be completed in full and
submitted with attachments to the foundation
- The sponsorship committee will review applications
- Applicants may be asked to for an interview or members of
the committee may conduct a site visit
- The board will review recommendations of the
sponsorship committee and make final determinations
- The applicant shall receive notice of the board's decision
in writing
- If the applicant receives a grant, an evaluation must be
submitted to the foundation not more than one year
following
